目的 總結(jié)因下肢深靜脈血栓形成(DVT)導(dǎo)致肺栓塞(PE)發(fā)生的概率,探討應(yīng)用腔靜脈濾器(VCF)植入預(yù)防PE發(fā)生的情況。
方法 回顧性分析我院2005年1月至2012年1月期間收治的1 058例下肢DVT患者的臨床資料。
結(jié)果 1 058例DVT患者中發(fā)生PE者34例(3.21%),發(fā)生死亡者15例(1.42%)。其中887例未植入VCF者中發(fā)生PE者32例(3.61%), 死亡15例(1.69%);171例植入VCF者中發(fā)生PE者2例(1.17%),無死亡患者。植入VCF的171例患者中經(jīng)股靜脈植入151例,經(jīng)頸靜脈植入20例;VCF植入后3~4周內(nèi)回收,嘗試回收145例,成功回收139例,其中經(jīng)頸靜脈回收13例,126例經(jīng)股靜脈回收。PE和死亡的發(fā)生時間均集中在10d內(nèi),PE發(fā)生最長時間為35d。未植入VCF患者的PE發(fā)生率及死亡率均明顯高于植入VCF患者
(P<0.01)。未植入VCF患者的PE發(fā)生率及死亡率雙下肢均明顯高于右下肢及左下肢(P<0.05),右下肢明顯高于左下肢(P<0.05)。下肢DVT合并下腔靜脈血栓者PE發(fā)生率及死亡率均明顯高于中心型(P<0.05),中心型明顯高于周圍型(P<0.05),周圍型與混合型間差異無統(tǒng)計學(xué)意義(P>0.05)。隨訪(39±19)個月(1個月~7年),VCF的累積通暢率為98.7%。植入的濾器無移位、傾斜和擴張不良。
結(jié)論 VCF植入能有效預(yù)防PE,但應(yīng)嚴(yán)格掌握適應(yīng)證。
